Try a short attention practice, without expecting it to cure ADHD.

**The number:** Very low certainty: large short-term gain in adults' self-rated symptoms (SMD 0.928) ([Gosling et al., BMJ accompanying open data, 2025](https://raw.githubusercontent.com/CorentinJGosling/EBI-ADHD-UR-2025/86eff22a9ffb/datasets/dataset-ur-adhd.csv))

## The evidence

- How sure: Certainty of evidence: Very low certainty. Measured in people with ADHD.
- See this comparison on the evidence map: https://adhd100.jock.pl/evidence/#r-mindfulness-adult-core-adhd-symptoms-self-rated-short
- Evidence grade: C. Grade C: small or mixed studies, expert consensus or clinical practice. Worth knowing, not worth betting on.
- Basis: EBI re-estimation: 4 adult trials, 264 participants; self-rated core symptoms at short-term endpoint, not a one-minute practice.
- Outcome: adult self-rated core ADHD symptoms after full mindfulness programmes
- Comparison: vs passive controls, including waitlist
- Benefit: 35/100. The benefit score is an editorial prioritization for the general list, weighted by certainty. It is not a prediction of what will happen for you.

## Why it matters

Full mindfulness programmes showed a large short-term improvement in adults' self-rated ADHD symptoms, SMD 0.928, but certainty was very low. The estimate came from small trials near 12 weeks; it does not establish lasting benefit or the effect of a brief exercise. The emotional-dysregulation result was inconclusive, so feeling calmer is something to notice rather than a promised outcome.

## How to do it

1. Get comfortable and notice a sound, or your breath as it is.
2. When your attention drifts, notice it and gently come back.
3. Give a brief practice a go before deciding if it suits you. Stop if it increases distress.

## Talk to a clinician first

Mindfulness is not helpful for everyone. Stop if it worsens distress, and discuss persistent symptoms with a clinician.

## Putting it into practice

### Start now

- 1 minute: Listen for a nearby sound or feel your natural breath. When your mind wanders, bring it back gently.
- 5 minutes: Try a short seated practice from the NHS guide, keeping your eyes open if you prefer.
- 15 minutes: Read the NHS mindfulness guide, try a brief exercise and notice whether it feels helpful or distressing.

### What it looks like

- Notice the sounds around you while sitting comfortably.
- Notice that your mind has wandered and return to the sensation of breathing.

### Common mistakes

- Trying to empty your mind sets a target you don't need. Practise noticing and coming back instead.
- A missed practice does not cancel earlier attempts, so resume only if it is useful to you.
