When 'lazy' pops into your head, describe what really happened and answer yourself the way you'd answer a friend.

**The number:** Self-compassion programmes, mostly outside ADHD: small to medium drop in stress (SMD 0.43) ([Mindfulness, 2023](https://pmc.ncbi.nlm.nih.gov/articles/PMC10239723/))

## The evidence

- How sure: Certainty of evidence: Not rated. Measured in the general population; we say so because ADHD trials of this do not exist or are too small.
- Evidence grade: C. Grade C: small or mixed studies, expert consensus or clinical practice. Worth knowing, not worth betting on.
- Basis: General-population and mixed clinical RCT meta-analysis with high overall risk of bias; NHS self-esteem guidance; NIMH for ADHD relevance. No GRADE rating or ADHD-specific effect.
- Outcome: stress reduction after self-compassion programmes, not a brief exercise
- Comparison: vs mixed active and passive controls; mostly not ADHD
- Benefit: 40/100. The benefit score is an editorial prioritization for the general list, weighted by certainty. It is not a prediction of what will happen for you.

## Why it matters

Across 30 trials, mostly outside ADHD, self-compassion programmes reduced stress at the end of treatment, SMD 0.43, but most trials had a high risk of bias and there was no clear advantage over active alternatives. It matters with ADHD because procrastination and trouble finishing tasks are common in adults with ADHD, and easy to mislabel as laziness. NHS self-esteem guidance suggests checking harsh beliefs against evidence and talking to yourself as you would to a friend. The one-minute version is a manageable start, not a proven ADHD treatment.

## How to do it

1. Notice a harsh label like 'lazy' or 'useless', and don't scold yourself for having it.
2. Say what actually happened, plus one fact that doesn't fit the label.
3. Give yourself the reply you'd give a friend. Then name one support or repair you need.

## Talk to a clinician first

Persistent worthlessness, hopelessness or thoughts of self-harm deserve prompt professional support.

## Putting it into practice

### Start now

- 1 minute: Say it silently: 'This is a hard moment. What would I say to someone I care about?'
- 5 minutes: Write one harsh belief about yourself and one concrete fact that challenges it.
- 15 minutes: Read the NHS self-esteem guide and write a kind, balanced reply to one criticism that keeps coming back.

### What it looks like

- After delaying a task: 'I find starting hard. Calling myself lazy will not tell me what support I need.'
- 'I missed a message' is fairer than 'I am a bad friend', and still leaves room to apologise.

### Common mistakes

- Forced positivity can feel false, so choose a fair and believable sentence.
- Kindness does not erase the effect on others, so pair it with a repair where one is needed.
