Make rules your child can understand, and respond the same way each time without escalating.

**The number:** Use clear rules and consistent responses ([NICE, public HSC Northern Ireland copy, 2018](https://cypsp.hscni.net/wp-content/uploads/ebpp/reports/attention-deficit-hyperactivity-disorder-diagnosis-and-management-pdf-1837699732933.pdf))

## The evidence

- How sure: Certainty of evidence: Very low certainty. Measured in people with ADHD.
- See this comparison on the evidence map: https://adhd100.jock.pl/evidence/#r-parent-mediated-behavioral-interventions-youth-oppositional-symptoms-odd-parent-rated-short
- Evidence grade: A. Grade A: moderate or high certainty evidence in people with ADHD, or an explicit ADHD guideline recommendation.
- Basis: Explicit NICE parenting recommendation; EBI oppositional-symptom estimate is for full parent-mediated programmes.
- Benefit: 41/100. The benefit score is an editorial prioritization for the general list, weighted by certainty. It is not a prediction of what will happen for you.

## Why it matters

NICE supports clear, appropriate rules and consistent responses for children with ADHD. Full parent-mediated programmes gave a small improvement in parent-rated oppositional symptoms near 12 weeks, SMD 0.249, but EBI certainty was very low. That result belongs to the whole programme, not any particular consequence, and whether the benefit lasts is uncertain.

## How to do it

1. Agree a small number of rules everyone can understand, at a calm moment.
2. Explain ahead of time what a proportionate response will be. Keep the words brief and calm.
3. Watch for confusion or overwhelm, and help your child practise what's expected.

## Talk to a clinician first

Avoid physical punishment, humiliation or withholding basic needs. Ask for professional help if family conflict or aggression is escalating.

## Putting it into practice

### Start now

- 1 minute: Say one calm, concrete boundary out loud: 'I will help you, and I will not let anyone get hurt.'
- 5 minutes: Choose one repeated conflict and decide how to explain the expectation in simple words.
- 15 minutes: Discuss one rule and a proportionate response with your child, then check that they understand.

### What it looks like

- Agree beforehand that a shared game pauses if someone starts hitting.
- Say what is expected in a short sentence, then allow time to process it.

### Common mistakes

- Changing the rule in anger makes it harder to understand, so return to the agreed expectation.
- Treating confusion as defiance can escalate things. Check they understood, and offer help.
